When elderly parents need care, families grapple with a host of challenges. Can mom and dad stay home? What types of programs and services do they need? How do I balance the demands of elder caregiving with the many other demands in my life?

Eldercare Resources is here to help every step of the way. In some cases, we are asked to help a family get a snapshot of the senior’s situation and identify resources, and then the family does the rest. In most cases, we continue to work with the family to implement the services the senior needs and to monitor his or her ongoing care. When you work with Eldercare Resources, you can be confident that your family has a trained and compassionate ally you can count on.

In-Home Assessment

Your care manager will complete a detailed in-home assessment that evaluates health and safety, legal and financial issues, medical history, ability to execute activities of daily living, and emotional and psychological functioning. The assessment includes a written report with recommendations and referrals to available community resources.

Care Plan

The care plan is a formal written document which summarizes the findings of the assessment, provides recommendations and referrals for care options. Our Geriatric Care Manager will go into great depth in explaining the details of the plan, what led to the recommendations, what you can expect as you put the care plan in place.

Ongoing Monitoring

Ongoing management of the needs of the elderly person may include: monitoring in-home help, serving as a liaison to families at a distance, and providing advocacy, counseling and support.
